ABDOMINAL AORTIC ANEURYSM (AAA)

Abdominal aortic aneurysm is the dilatation of abdominal aorta,
the most common site of such aneurysm is the Infra – renal aorta.
This is not an uncommon condition and involves 5% of the male
population who are more than 60 yrs of age.
In female population it is much less common but still is seen
in approximately 1.8 % of population who are more than 60 years
of age.

More then 50000 abdominal aortic surgeries are performed in the USA, while in our vast country less then 200 operations are performed on the whole aorta at very few limited centres

Usually remains asymptomatic & rupture or death are usually the first symptoms Not suspected or not palpated, most commonly detected on an Abdominal Ultra sound for other reasons

Treatment (Surgical Or Endovascular) is needed for males if AAA is more than 5.5 cm in size and for females if more then 5.0 cms in size.

Thoracic Aortic Aneurysm & Dissection The aorta is the largest artery in the body, and it carries blood away from the heart to all the parts of the body. The part of aorta that runs through the chest is called the thoracic aorta and, when your aorta reaches your abdomen, it is called the abdominal aorta. When a weak area of your thoracic aorta expands or bulges, it is called a thoracic aortic aneurysm (TAA). These aneurysm can be treated either by a non surgical technique where a stent graft can be inserted through a small cut in the leg and the aneurysm excluded with out any operation but this can be done only in some suitable patients if the non surgical ( endovascular treatment) is not found suitable the operative surgical treatment is equally successful. aa110

CASE

Mr. K N a 63 years old man who was diagnosed to have acute dissection of thoracic aorta and was told that he would not survive and die with in next 48 hrs, took a chance and traveled almost 36 hrs to reach and was successfully treated with Endovascular stenting where an stent graft was inserted and doing very well after 6 months.

Certain conditions of the heart are declared inoperable.

Like this middle aged man from Tamil Nadu with aneurysm of aorta  

Mr. Muthuraman (name changed) had aneurysm of arch of aorta, which eroded the sternum and pulmonary artery causing aorta pulmonary fistula. When he presented with severe respiratory distress and swelling in the chest wall, an enthusiastic young doctor stuck as needle into the swelling to aspirate. When red blood started squirting, he was promptly

referred to our hospital with his wife controlling the bleeding by a handkerchief.

He had an emergency surgery under cardiopulmonary bypass (femerofemoral route under local anesthesia) since it was not possible to intubate the compressed trachea. His arch vessels were replaced with a haemashield tube and pulmonary artery was repaired with a pulmonary homograft. He left the hospital after 2 weeks and now working as a full time labourer.

Under CPB and hypothermia it is possible to replace the thoracic aorta, re-implant spiral arteries to prevent paraplegia, re-implant both renal arteries and mesenteric arteries with virtually no blood transfusion.heart
