Centers of Excellence - Pulmonology

Pulmonology is a sub-specialty of Internal Medicine that deals with the health of the respiratory tract. In particular, the health of the lower airway, the lungs and the control of breathing, as well as their effects on the oxygenation of blood are the focus of the sub-specialty.
The sub-specialty is also referred to by other names, including Respiratory Medicine, Thoracic Medicine, Chest Medicine, and rarely, Respirology. Pulmonology also has extensive overlap with Critical Care, and along with Anaesthesia and Internal Medicine contributes to the core competencies in the sub-speciality.

The department of Pulmonology is dedicated towards
  • providing comprehensive care for all respiratory disorders in the Narayana Health City setting
  • bringing the best and latest evidenced based respiratory care to our clients
  • promoting respiratory health in the community
  • participating in respiratory health education at all levels
  • actively participating in and facilitating research into respiratory disorders in the community
Narayana Hrudayalaya, initially and primarily developed as a hospital catering to the needs of those with cardiac disease, has a large number of its clients who need the services of the Pulmonology Department. As several symptoms are common to respiratory and cardiac diseases, evaluation of the person with symptoms such as breathlessness occupies a major portion of the work of the department. Respiratory disease can also commonly occur co-morbidly with cardiac disease; the strong association of Chronic Obstructive Pulmonary Disease (COPD) with coronary artery disease is the best example. Holistic care of such persons necessitates the association in their care, of the Cardiology and Pulmonology teams.
The Department of Pulmonology has been continuously functioning from 1st June 2006. Initially having a single consultant, the department now has 2 Pulmonologists, and a thoracic surgeon.

Outpatient Services:
Evaluation and management of new, review and referred patients, for
a. Evaluation of respiratory symptoms and diseases
b. Optimization of respiratory function pre-operatively
c. Respiratory evaluation as part of an overall annual health-check.

Pulmonary Function Laboratory:As a part of the Pulmonology service, the department runs a Pulmonary Function Laboratory, serving both outpatients and inpatients. Spirometry is also an essential part of the pre-operative work-up. Pulmonary Function Services are planned to be expanded to include diffusion studies, and lung capacities.

Bronchoscopy:Fibreoptic bronchoscopy is performed for adults, both for out patients and in patients.
A Sleep Laboratory, comprising of polysomnography using a Sandman Elite system is in operation since 10 months. The Pulmonology consultants also interact closely with the neurology consultants in the diagnosis and management of sleep related problems.

Emergency Care:Pulmonology emergencies are managed in the MICU primarily by the pulmonologists and intensivists, while Pulmonology consultations for other admitted patients in all wards including the ICUs are offered around the clock .

Training:The department of Pulmonology is run inseparably from the department of Internal Medicine, which in turn conducts he DNB (General Medicine) programme. Training of medical residents in the DNB programme is therefore an integral part of the daily work of the department, and includes a significant research component. Teaching programs for other doctors, nurses, physiotherapists and technicians are conducted both formally (lectures and demonstrations) and informally (teaching on daily rounds). The department also participates in a holistic medical care program with training of residents from the Yoga, Ayurveda and Naturopathy streams working towards their MD or PhD programs at VYASA a deemed University dedicated to education and research in the complementary systems of medicine.

Continuing Medical Education:
a. The Pulmonology consultants make every effort to keep abreast of the State of the Art in Pulmonology, with regular attendance at national and international conferences in Pulmonology, Bronchology and Internal Medicine.
b. CME programs are organized on various Pulmonology topics for general practitioners, physicians and pulmonologists.
c. The pulmonologists in the department were recently invited to participate in a symposium at the prestigious APICON 2009 at NOIDA, and contribute a chapter to the accompanying, widely read Medicine Update.

Dr Murali Mohan has been awarded the Dr Nadagouda oration at the KAPICON 2009, the state physicians conference.

The Consultants:
Dr B V Murali Mohan graduated from the Bangalore Medical College in 1982and obtained his MD (General Medicine) from the same institute in 1986. He obtained the MRCP (UK) by examination in 1992 and then trained in Respiratory Medicine at the Kings Cross and Nine Wells Hospitals (Dundee) and Newcastle General Hospital (Newcastle upon Tyne). He was on the faculty of the Department of Medicine at Dr B R Ambedkar Medical College, at various positions including Professor and Head of the department. He has been at Narayana Hrudayalaya since June 2006. His special interests are in asthma, COPD, epidemiology of cardiovascular diseases and cardio-pulmonary resuscitation, in which areas he has several papers, publications and invited lectures at national and international level. He underwent Pulmonology training at Mysore Medical College (DTCD) and Mediciti Hospital, Hyderabad (DNB) and worked as senior resident at LRS institute of Tuberculosis and Allied sciences, New Delhi, an apex institute for tuberculosis. His areas of special interests are respiratory intensive care, interventional Pulmonology and sleep medicine.
